Das von diesem Gen kodierte Protein ist das erste Enzym des Fettsäure-Beta-Oxidationsweges, das die Desaturierung von Acyl-CoAs zu 2-trans-Enoyl-CoAs katalysiert. Es spendet Elektronen direkt an molekularen Sauerstoff und produziert dabei Wasserstoffperoxid. Defekte in diesem Gen führen zu pseudoneonataler Adrenoleukodystrophie, einer Krankheit, die durch die Akkumulation von sehr langkettigen Fettsäuren gekennzeichnet ist. Alternativ gespleißte Transkriptvarianten, die verschiedene Isoformen kodieren, wurden identifiziert.
