ALT katalysiert die Amino-Umwandlungsreaktion zwischen Alanin und α-Ketoglutarsäure unter Bildung von Brenztraubensäure und Glutaminsäure. Unter der Wirkung der Pyruvatoxidase erzeugt Brenztraubensäure H2O2, das unter der Wirkung der Peroxidase mit einer nicht-fluoreszierenden Substanz reagiert, um eine fluoreszierende Substanz zu bilden. Die Aktivität von ALT kann durch Messung der Zunahme des Fluoreszenzwertes bei der Anregungswellenlänge von 535 nm und der Emissionswellenlänge von 590 nm berechnet werden.
